Rural GP surgeries frequently experience struggles in both hiring and keeping the staff members needed for their multidisciplinary teams. A scarcity of research currently exists concerning rural recruitment and retention, often centering on the recruitment and retention of medical professionals. Rural areas frequently depend on the revenue streams from dispensing medications, yet the contribution of consistent dispensing services to the recruitment and retention of personnel is not fully researched. This study sought to investigate the obstacles and catalysts for continuing employment in rural pharmacy practices, along with exploring the primary care team's appreciation of dispensing services.
England's rural dispensing practices were the focus of semi-structured interviews with their multidisciplinary team members, which we undertook. Interviews were conducted via audio, and these recordings were subsequently transcribed and anonymized. Employing Nvivo 12 software, a framework analysis was carried out.
Twelve rural dispensing practices in England, each employing seventeen staff members (general practitioners, practice nurses, managers, dispensers, and administrative staff), were subjected to interviews. Individuals considering a role in rural dispensing were drawn to both the personal and professional advantages, which included a high degree of career autonomy and professional development prospects, coupled with the appeal of rural living and working. Revenue from dispensing, opportunities for skill enhancement, satisfaction in their roles, and a constructive work setting all contributed significantly to staff retention. Factors impeding retention included the mismatch between required dispensing expertise and offered salaries, a scarcity of qualified applicants, transportation issues, and an unfavorable perspective on rural primary care roles.
The drivers and challenges of working in rural dispensing primary care in England will be better understood through these findings, which will consequently inform national policy and practice.

The Aboriginal community of Kowanyama is situated in a remarkably secluded area. Among Australia's top five most disadvantaged communities, there is a high and heavy burden of disease associated with it. GP-led Primary Health Care (PHC) serves a population of 1200 people 25 days a week. This audit investigates whether general practitioner availability is linked to patient retrievals and/or hospital admissions for potentially preventable conditions, exploring its cost-effectiveness and effect on outcomes, while striving for the implementation of benchmarked GP staffing levels.
An in-depth analysis of aeromedical retrievals in 2019 was undertaken to determine if rural general practitioner access could have mitigated the need for retrieval, evaluating each case as 'preventable' or 'non-preventable'. A comparative cost analysis was conducted to assess the expense of achieving standard benchmark levels of general practitioners within the community versus the cost of potentially avoidable retrievals.
2019 saw 89 retrieval procedures performed on 73 patients. A significant portion, 61%, of all retrievals were potentially avoidable. No doctor was on the premises for 67% of the preventable retrieval events. Retrieving data about preventable conditions resulted in more clinic visits from registered nurses or health workers (124) than for non-preventable conditions (93), while general practitioner visits were fewer for preventable conditions (22) compared to non-preventable conditions (37). For 2019, the conservatively calculated retrieval costs were the same as the maximal expense for benchmark data (26 FTE) for rural generalist (RG) GPs using a rotational structure in the audited community.
The increased availability of general practitioner-led primary healthcare in public health facilities seems to result in fewer requests for transfer and fewer hospitalizations for potentially preventable conditions. The consistent on-site availability of a general practitioner is likely to mitigate the number of preventable condition retrievals. Establishing a rotating system for RG GPs in remote areas, coupled with benchmarked numbers, is a cost-effective way to improve patient health outcomes.
It seems that readily available primary healthcare, with general practitioners at the helm, contributes to fewer cases of patient retrieval and hospital admission for possibly preventable ailments. Amidst deep uncertainty, the initial phase of the COVID-19 pandemic presented a crisis, an immediate and urgent threat requiring decisive intervention. During the data collection process, eight municipal chief medical officers of health (CMOs) and six crisis management teams were engaged in semi-structured and focus group interviews. Through systematic text condensation, the data were subjected to analysis. Inspiration for the analysis stemmed from Boin and Bynander's approach to crisis management and coordination, and from Nesheim et al.'s proposed framework for non-hierarchical coordination within the state apparatus.
Rural municipalities enacted local infection control protocols due to the compounding anxieties of a pandemic with unknown repercussions, inadequate infection control supplies, difficulties in transporting patients, the precariousness of their healthcare workforce, and the necessity of securing local COVID-19 bed capacity. The engagement, visibility, and knowledge of local CMOs fostered trust and safety. Differences in the standpoints of local, regional, and national parties generated a tense situation. Existing roles and structures were adapted, and novel informal networks emerged.
