Adverse drug reactions (ADRs) represent a substantial worldwide public health issue; nevertheless, pharmacovigilance systems are plagued by a lack of reporting. Mobile apps, such as Med Safety, and other mobile technologies, could augment the reporting of adverse drug reactions. In Uganda, we investigated the acceptability of the Med Safety system for adverse drug reaction reporting among healthcare workers and factors influencing its implementation.
In twelve HIV clinics situated within Uganda, a qualitative, exploratory research design was implemented during the period from July to September 2020 for this study. Our research included 22 in-depth interviews and three mixed-gender focus groups with 49 participants from diverse backgrounds within the healthcare profession. Our data analysis was approached thematically.
There was a notable willingness among medical professionals to integrate Med Safety for adverse drug reaction reporting, and the majority felt it was worthy of recommendation to their colleagues. Application acceptance grew in tandem with the implementation of training exercises. The younger, tech-adept health workforce embraced the app, spurred by its offline communication tools, two-way feedback mechanism, readily accessible Wi-Fi in certain facilities, the willingness of healthcare staff to document adverse drug reactions, and the complex procedures of conventional ADR reporting systems. Obstacles to Med Safety implementation included the time-consuming initial app registration and multiple screens required for ADR reporting. Health worker smartphone challenges (incompatibility, limited storage, low battery), along with high internet data costs, poor connectivity, ADR identification issues, language barriers, and poor feedback mechanisms, were all significant barriers to broader adoption.
There was a positive reception amongst health professionals regarding the implementation of Med Safety for ADR reporting, with a substantial portion recommending the app to fellow health workers. App acceptability, boosted by practice-based training, should be a cornerstone of all future app deployments. An investigation into the repeatability of corneal pachymetry and epithelial thickness, obtained through spectral-domain optical coherence tomography (SD-OCT), will be conducted, along with an exploration of correlations between these measurements and ocular surface parameters.
Volunteers who engaged in lengthy computer sessions were enrolled, excluding those with conditions impeding corneal measurements or tear production. Without exception, all subjects filled out the ocular surface disease index (OSDI) questionnaire. Consecutive central and peripheral corneal and epithelial thickness measurements were acquired via SD-OCT (RTVue XR) in triplicate. A study of Schirmer test I and tear film break-up time (TBUT) was completed. Repeatability was established through the application of intraclass correlation coefficient (ICC), coefficient of variation, and repeatability limit. For the study, 113 eyes belonging to 63 subjects were selected for examination. Rarely, inflammatory bowel disease presents with aseptic abscesses outside the intestines. This report details a case of a 69-year-old female patient suffering from ulcerative colitis, in whom multiple aseptic abscesses responded favorably to infliximab treatment. Aseptic abscesses secondary to ulcerative colitis can be indistinguishable from infectious abscesses in terms of their presentation. A diagnosis of aseptic abscesses, complicating ulcerative colitis, was established in this clinical presentation. The failure of antibiotics and subsequent negative results from repeated Gram stains and cultures of both blood and abscess material confirmed the diagnosis. Aseptic abscesses, while commonly found in the spleen, lymph nodes, liver, and skin, were predominantly situated in the periosteum in the presented case.
