Indonesia's National Health Insurance (NHI) mechanism has fostered substantial progress towards universal health coverage (UHC). In the context of the Indonesian NHI program, socioeconomic stratification led to diverse levels of comprehension regarding NHI concepts and procedures among different population segments, thereby increasing the chance of disparities in healthcare access. This investigation utilized the secondary dataset from the 2019 national survey on 'Abilities and Willingness to Pay, Fee, and Participant Satisfaction in implementing National Health Insurance in Indonesia,' a survey conducted by The Ministry of Health of the Republic of Indonesia. The study population encompassed a weighted sample of 18,514 impoverished individuals from Indonesia's populace. The dependent variable, NHI membership, was examined in the study. The study delved into seven independent variables—wealth, residence, age, gender, education, employment, and marital status. The final phase of the analysis involved the application of binary logistic regression.
Statistical results highlight a trend wherein NHI membership is more prominent among the financially disadvantaged with advanced educational qualifications, residing in urban environments, being older than 17, being married, and having higher financial stability. For the impoverished segment of the population, a higher level of education is a significant predictor of NHI membership, compared with those having lower educational levels. Their NHI membership was also influenced by details including their residence, age, gender, job, marital status, and overall financial situation. Poor individuals holding primary education are significantly, 1454 times more likely to become members of NHI, as compared to those devoid of any formal education (AOR = 1454; 95% CI: 1331–1588). Those who have completed secondary education are 1478 times more predisposed to being members of the NHI than individuals with no formal education, as indicated by the analysis (AOR 1478; 95% CI 1309-1668). The likelihood of NHI membership among the impoverished populace is significantly influenced by variables including educational background, residential location, age, sex, employment status, marital standing, and economic status. The disparity in predictors amongst the poor, according to their educational levels, strongly influences our findings, which emphasize the critical importance of government investment in NHI, and the necessity of concomitant investments in education for this population.

The identification of clusters and related factors within physical activity (PA) and sedentary behavior (SB) is critically important for developing tailored lifestyle programs for children and adolescents. Through a systematic review (Prospero CRD42018094826), the study sought to determine the clustering characteristics of physical activity and sedentary behavior, and the contributing factors, amongst boys and girls aged 0 to 19. The investigation employed five electronic databases in its search. With the authors' specifications as a framework, two independent reviewers extracted cluster characteristics. A third reviewer settled any resulting disagreements. The population of seventeen studies included children and adolescents, ranging in age from six to eighteen years. Mixed-sex samples exhibited nine, boys twelve, and girls ten distinct cluster types. Groups of girls were characterized by both low physical activity and low social behavior, and also by low physical activity and high social behavior. In contrast, a significant proportion of boys were found in clusters marked by high physical activity levels and high social behavior, and high physical activity levels with low social behavior. There were few discernible links between sociodemographic factors and all the identified cluster types. High PA High SB clusters presented elevated BMI and obesity levels in both boys and girls, across most examined associations. Alternatively, the High PA Low SB clusters displayed lower BMIs, waist circumferences, and a smaller proportion of overweight and obese individuals. A comparison of boys and girls revealed differing cluster patterns for PA and SB. The High PA Low SB cluster demonstrated a more favorable adiposity profile in children and adolescents, regardless of their gender. The outcomes of our study imply that an elevation in physical activity levels is not sufficient to control the indicators of adiposity; a concomitant reduction in sedentary behavior is also necessary for this particular demographic.
With the reconfiguration of China's medical system, Beijing municipal hospitals experimented with a novel pharmaceutical care model, establishing medication therapy management services (MTMs) in their outpatient clinics from 2019. Our hospital, one of the first in China, established this service. The reports available concerning the impact of MTMs in China, as of this moment, were relatively few in number. This study documents the implementation of medication therapy management (MTMs) at our hospital, explores the potential of pharmacist-led MTM programs in outpatient settings, and analyzes the consequences of MTMs on patient healthcare expenditures.
In Beijing, China, a university-connected, comprehensive tertiary hospital served as the site for this retrospective analysis. Patients documented with complete medical and pharmaceutical files, having received one or more Medication Therapy Management (MTM) services from May 2019 through February 2020, were part of the study group. Under the guidance of the American Pharmacists Association's MTM standards, pharmacists delivered patient care focused on pharmaceuticals. This process included identifying the specific and categorized patient concerns about medication, diagnosing medication-related problems (MRPs), and developing practical medication-related action plans (MAPs). Pharmacists documented all identified MRPs, pharmaceutical interventions, and resolution recommendations, and calculated the cost of treatment drugs that could be reduced by patients.
In an outpatient context, MTMs were administered to 112 patients; 81 of these patients, having complete records, constituted the study population. Of the total patient population, 679% experienced five or more distinct medical conditions, and of this group, 83% concurrently used more than five medications. In a Medication Therapy Management (MTM) study of 128 patients, the patients' perceived medication-related demands were recorded. The most frequent demand concerned monitoring and evaluating adverse drug reactions (ADRs), comprising 1719% of the total. A total of 181 MRPs were identified, averaging 255 MPRs per patient. Adverse drug events (1712%), nonadherence (38%), and excessive drug treatment (20%) comprised the top three MRPs. Intricate patient care needs and a scarcity of nursing staff members are substantial issues faced by healthcare professionals working in nursing homes. Hence, nursing homes are undergoing a transformation to become personalized home-like facilities that focus on patient-centred care. Despite the imperative for an interprofessional learning culture in nursing homes, as necessitated by various challenges and shifts, the driving forces behind its development are not fully grasped. The facilitators are the focus of this scoping review, which aims to uncover the contributing elements to their identification.
In compliance with the JBI Manual for Evidence Synthesis (2020), a scoping review was performed. The search, spanning the 2020-2021 timeframe, leveraged seven international databases: PubMed, Cochrane Library, CINAHL, Medline, Embase, PsycINFO, and Web of Science. Two researchers separately identified the reported facilitators contributing to interprofessional learning climates in nursing home settings. The researchers then inductively categorized the extracted facilitators into groups.
The comprehensive search unearthed 5747 studies. This scoping review included 13 studies, which met the pre-defined inclusion criteria, subsequent to the removal of duplicate entries and the screening of titles, abstracts, and full texts. The 40 facilitators were organized into eight categories: (1) shared language, (2) shared objectives, (3) explicit tasks and responsibilities, (4) knowledge dissemination and acquisition, (5) teamwork-based methods, (6) change and innovation championed by the front-line supervisor, (7) openness and inclusivity, and (8) a safe, courteous, and transparent work environment.
With the goal of evaluating and identifying areas requiring enhancement within the current interprofessional learning culture in nursing homes, we found suitable facilitators.
